Vendor Registration

Thank you for your interest in becoming a vendor. To register as a new vendor with BCHA, please complete the Vendor Registration form and submit it to BCHA. Before a purchase order or contract is extended to you, you must be able to furnish the following information:

  1. Copy of Current business license
  2. Certification of Insurance
  3. IRS Form W-9
